SKIN – Cracked heels 

 

Our skin is a good barometer which indicates how healthy the body is. Skin acts as a covering and works as a defense force against infection. It protects our body from heat or cold, external germs, noxious chemicals or toxins, etc. The most common friction spots harden to give additional protection against injury, whereas other parts remain soft. Dry parched skin indicates its poor quality.

Cracks mean initial, partial break or interruptions in continuity. Cracks are also medically termed as fissures. Fissures are regular linear cut wounds – mostly superficial which involve epidermis. Sometimes it may get deep into the dermis and become painful.

Cracks in the skin can arise anywhere in the body. The commonest site is the rim of the heel and web of toes, which are the most frequently or constantly used parts of our body. The cracks usually occur on the edges or at the junction of two different types of skins, for example the rim of the heel, corners of the mouth, lips, anus, nipple, webs of the fingers, ear folds, etc. Skin problems cause physical and psychological stress

 

Incidences

 

Cracks in the heel or heel fissures are most commonly seen in older people or persons who constantly walk barefoot. It can affect either both the heels or one of the heel. But most commonly, it is seen on both the heels. Likewise, fissures are more common with diseases like diabetes, eczema, psoriasis, etc. Females suffer most commonly than males, “with more indulgence in water, causing cracks”. But, unfortunately, people living in dry climates have higher incidence than persons in wet regions.

 

Causes – The occurrence of cracks may be due to external or internal factors. But dry skin or dryness is the prime cause.

 

External factors

·         Pressure and frequent movements

·         Standing for a long time

·         Change in walking posture

·         Summer heat

·         Over or constant indulgence in water

Unhygienic circumstances or conditions

 

 

 

Internal factors

·         Unhealthy, dry scaly skin due to climate or diseases

·         Thick dry scaly skin with loss of elasticity due to ageing – cracks have higher incidence over age

·         Deformity of foot due to congenital problem or injury or diseases – Flat foot, calcaneal spurs, etc

·         Diseases like diabetes, eczema and psoriasis can also contribute to cracked feet

·         Deficiency of vitamins, minerals and zinc

Obese (everyone can notice cracks on an elephant’s foot due to overweight and dryness) 

 

Symptoms

Cracks in the rim of the heel or soles or web of toes are the main symptoms. The length and width may vary depending upon the position and pressure exerted. The edges of the cracks are usually raised as crusts and resist healing. Other than fissures, mostly there won’t be any symptoms. But in extreme cases, pain will be more with bleeding and restriction of movement. Also, there will be pain in the cracks while walking or standing or touching the part. Red raw part may show in some deep ulcers. Discharges can occur in case of infection.

 

Preventive measures

Caring for skin

·         Keep soles clean

·         Wear soft, cushioned, comfortable shoes or slippers

·         Wear socks to prevent friction

·         Apply oil, if there is dryness

·         Take rich sources of vitamins – fruits, dates, leafy vegetables etc.

Avoid

·         Walking bare foot

·         Tight shoes

·         Standing for a long time in water

·         Contact of bleaching powder or any other chemical

Using strong chemical soaps

 

Self management

 is mainly for cushioning the skin to protect it from further damage and for moisturising or lubricating to promote the healing process in a better way.

 

 

 

Clean and keep it soft

·         Wash properly at bed time and make sure that sand or mud particles are not present inside the cracks

Make it dry for 10-15 minutes and then apply moisturiser cream or vaseline or oil, since immediate application of vaseline, after washing, will trap water inside the cracks and won’t let them to dry speedily. Also water inside the cracks can promote infection


Avoid for better healing

·         Sporting activities during the healing process

·         Barefoot walking to avoid mud getting into the cracks

·         Any friction or injury or chemical contact

·         Picking or peeling of skin since it may become red and sore

·         Sour foods, non-veg, fast foods, oily foods


For better healing

·         Do skin scrapping to reduce the thickness of the skin

·         Soak the soles in lukewarm water for 10 minutes and scrap mildly to reduce thickness. Some rub the foot against the floor or use stones or scrubber or loofah (mesh from the vegetable) while bathing, with the same result

·         Don’t be cruel in removing the thick skin to prevent injury

Apply petroleum jelly or vaseline or parraffin oil after drying the area


Complications

 Usually, there won’t be any complications. But when the cracks are deep and sufferer is diabetic, then there lies the danger of bleeding, infection and serious complications. So cracks must be attended to in the initial period itself or otherwise it may extend the damage to the internal organs of the body like the crack in a building ruining the whole structure when left un-repaired. Also it is often noticed that suppressing the cracked heels with steroidal ointments will lead to respiratory or asthma complaints. The relationship is not clearly known.


General treatment

 

In Allopathy, mostly internal medicines are not prescribed. Only in extremely painful deep cracks, antibiotics are used to control the infection. But the course of antibiotics often manages the situation temporarily and the cracks recur once you stop the medicines. External applications like germicidal or bactericidal or antiseptic or steroids are also commonly prescribed. These applications also clear the complaint very temporarily i.e. while in use. Vaseline or liquid paraffin is commonly advised for moisturising.

Treating dry skin externally – worthy?

For dryness of skin, in the startup phase, sufferers usually go for home treatment with the application of oil, butter, cucumber, lemon, honey, aloe vera juice, peppermint, etc according to their knowledge or ancestors advice. When complaints go out of control they will go to physicians or cosmetologists to get prescribed of ointments / oils / lotions. All the way, every one is most interested with external applications than with internal medicines. With topical applications, everyone can see the resurface of complaints in due course of time or with the stoppage of applications. Sometimes steroidal ointments may need to be applied in continuous manner for suppressing the dryness or skin complaints.

Whether this approach is correct????

Here, one need to aware that the disappearance of the scaly skin / itching doesn’t mean that the disease is waning or getting cured, i.e., skin rash can be easily made to disappear with steroidal external applications which will usually mask the complaint very temporarily. This palliative type of treatment usually drives the disease inner or deeper. Dry Skin

Dryness of skin is a very common skin problem and everyone might have experienced it in one or other time. It is medically termed as “Xeroderma”. This annoying dry skin may occur temporarily as a reaction to environment / irritants or in persistent manner with various systemic illness or skin complaints.

 

Incidences

 it is most common with babies and elderly peoples. Dry skin is usually felt more in extremities (legs and hands) and face / head.

 

Symptoms 

 dryness of skin usually leads to scaling, itching, redness, cracks and infection. In course of time, skin will turn rough and ugly.

Causes  are innumerous starting from simple dehydration.

Our skin is a good barometer of how healthy the body is. A frequent recurrent infection of the skin may indicate that the immunity system is weak. Skin has its own interference with lifestyle in colour and texture. Our skin controls and regulates the water content of the body. It protects body as a covering and works as defence force against infection. It is helpful in activating vitamin D when it is exposed to sunlight. If skin is damaged water content will be lost. Infection will spread resulting in renal shut down and collapse.

When skin or body is confronted with any other foreign substance, our immune system counteracts to withstand any ill effects from that substance. Antigen-antibody reaction will develop to inhibit all the ill effects. If this is excessive and it creates disease like condition then it is called Allergy. So the heightened reaction of the body to external agent is called Allergy. Antigens developed from this reaction attach to blood cells and form mast cells. When this gets contact with allergen, it secretes histamine which is primary chemical conductor and responsible for allergic reactions. When the body has been exposed to the elements known as allergen, which has been previously exposed, allergies form on the body. Allergies are different types as allergic rhinitis, sinusitis, asthma and allergic skin disorder. All allergies are serious and very uncomfortable. Allergies can be due to any foreign material –natural or artificial.

If any foreign substance enters into the nose, we sneeze. If this occurs continuously as allergy then it is called as allergic Rhinitis. Likewise, if there is any irritation in the skin, body reacts as red itchy swelling initially, which in due course with subsequent exposure leads to allergic skin disorder.

Skin allergy usually subsides by it self, and denotes about the bodies sensitivity. If this is excess it becomes allergic skin disorder.

Skin allergies or Allergic Skin Disorders are a serious and uncomfortable type. Any red, bumpy, scaly itchy, swollen skin can signify an allergic skin condition. This is the result of immune reaction against an irritant. From an overall healthcare the problem of allergy scare is dismissed as minor annoyances without due consideration for diagnosis and treatment. The majority sufferers are females leading to dismissal more readily than if the symptoms were experienced by men. Allergy in some form occurs in all individuals.

SYMPTOMS OF SKIN ALLERGIES


Allergic skin condition can take several forms and are due to various causes. They are:

Urticaria — Urticaria is a smooth flat red eruption with white center called Hives. It appears, disappears or reappears suddenly, without leaving any trace. It causes itching, burning and a stinging sensation anywhere in the body. Acuteness may be caused due to scratching, pressure, aggravation on the nerves, exercises or physical exertion, direct exposure to sunlight or cold , metal, pollens, plants, chemicals, oils cream, food as crab, oyster, prawn, tomato, lime and drugs like aspirin, anti inflammatory drugs, BP medicines and pain killers. Hives are a result of histamine acting on the upper layers of skin. Histamine is primary chemical mediator caused due to antigen-antibody reaction.


The urticaria may be as:

·         Heat urticaria-allergy caused due to heat

·         Papular urticaria-allergy caused due to encounter of flies or mites

·         Solar urticaria- a rare form of hive due to exposure to UV radiation

·         Cold urticaria-allergy caused due to cold

·         Pressure urticaria- allergy caused due to tight dresses or any tight situation.

 

Allergic Contact Dermatitis 

 When skin comes in contact with an allergic substance as plants, metals, dyes, rubber products, cosmetics and medications, a reaction starts after 1 to 3 days leading to red, itchy, inflamed skin known as Allergic Contact Dermatitis. It is more painful than itchy when the agent in contact with skin is more concentrated. Cold soaks and compresses can offer a relief. All age groups can be affected by contact dermatitis and potential triggers may be found just about anywhere. The hands and feet are the most common areas for contact dermatitis to occur. Usually, these symptoms will occur only in the areas that actually came into contact with the irritant or allergen.

 

Sun Rash 

Some people have sensitive skin to the sun in spring and summer. For them, exposure to sunlight results in a rash which may recur throughout summer. These are small reddish blisters in areas exposed to sunlight. They are extremely itchy. There is relief only during winter. For young ones it is very recurrent since skin is very weak and it disappears as they get older.


Angioedema

 Angioedema is a swelling resulting from histamine action on deeper layers of skin and it may or may not occur with hives. It is not red or itchy and mostly affects or occurs in soft tissues such as eye-lids, mouth or genitals


Atopic Dermatatis (Eczema)
 

Atopic Dermatatis is allergic eczematous condition affecting mostly face, elbows and knees. Itching leads to scales and flakes. In intense cases, oozing will occur if a bacteria or viral infection is introduced by scratching. Common causes are allergens, sweating, overheating, and emotional stress and strain, and diet, irritants in the wool, pets, soaps, and other agents. This is most common in children and individuals with asthma and allergic rhinitis.

DIAGNOSIS OF SKIN ALLERGY

A correct diagnosis along with proper therapy instituted early in the course of the disease will minimise frustration. The diagnosis of skin allergy can be made through:

Intestinal irritation-due to foods like crab, oyster, tomato and lime. Food allergies are usually more difficult to determine. Hives can arise 12-24 hours after consuming the food or food additive making it hard to recall what was eaten earlier. External irritation-due to use of oil and creams, Emotion, Climate changes Menstrual trouble, Suppressed sweat, Medications.


PREVENTIVE MEASURES FOR SKIN ALLERGY

About half the number of patients with chronic hives will have at least one more episode of hives in their lifetime. The primary treatment for eczema is that the patient should stop scratching and rubbing the rash. If any food is identified allergic it should be eliminated from diet. Use sunscreens for sun protection for photosensitive skin.


TREATMENT

Treatment also includes avoidance of stimulus, desensitization and stress reduction.

In allopathy, usually anti-histamines, nerve inhibitors are used to control allergy. It is temporary and works for only 12- 36 hours. This can be noticed in patients who are in continuous exposure to allergen — for example-occupational diseases. If patient takes drugs on long term, side effects will also add to problems.

Elderly people usually develop many complaints and often need to live with it with the help of medicines. The persisting nature of disease and dependency of medicines can deteriorate them further in mental health with impinge on hope. Good quality of life is very much essential in old age or otherwise more than suffering to themselves, they will also become a burden to their family. So, with the fear of disease, they will be perfectly taking the list of medicines prescribed.

          Medically means declining memory. It may be rapid or have slow progression. Sufferers of    

         dementia often miss their keys, umbrellas during start-up of the disease and later with

         persons, etc. 

·         Alzheimer's disease      

     It  is a progressive and degenerative disease of brain cells to cause forgetfulness.
